This can happen if the sender of the transaction creates multiple conflicting transactions and tries to get them confirmed simultaneously. Miners can only include one of these conflicting transactions in a block, leaving the others unconfirmed. This creates a risk for the recipient, as they may not receive the expected funds if the conflicting transaction gets confirmed instead. To mitigate this risk, it is advisable to wait for a sufficient number of confirmations before considering a transaction as final.
